Title: FNP
HOURS: Monday - Friday, 8 AM - 5 PM or 9 AM - 6 PM; one admin day (WFH) or two half days offsite for training and charting
Location: New York, NY 10001
SUMMARY: The FNP will work in a collaborative, team-based setting, seeing patients across all age groups. The role includes providing primary care, managing patient follow-ups, and coordinating with medical assistants, social workers, and other healthcare professionals.
DUTIES:
Provide comprehensive primary care services
Collaborate with team leaders and other healthcare providers
Perform assessments, develop treatment plans, and manage follow-up care
Participate in on-call phone triage on a rotational basis every 5-6 weeks (one week after hours, M-F and weekend, phone triage)(there is no additional compensation for on call, this is part of the duties)
MINIMUM QUALIFICATION:
Valid NY state license and DEA certification, Board Certified
2+ years of experience as Nurse Practioner
Experience with EMR systems (Athena preferred)
Bilingual: Spanish
QUALIFICATION:
Ability to work independently and collaboratively
Spanish language skills preferred but not required
Strong interpersonal and communication skills
