Dependable Health Services is seeking a compassionate and dedicated Hospice Medical Social Worker to provide essential psychosocial support to hospice patients and their families. This role plays a vital part in helping individuals navigate end-of-life care with dignity, respect, and comfort.

Assessment: Evaluate the psychosocial needs of patients and families and communicate findings to the interdisciplinary care team
Counseling: Provide emotional support, crisis intervention, and coping strategies for patients and caregivers
Education: Assist with advance directives, explain patient rights, and connect families with community resources
Collaboration: Establish and maintain relationships with community agencies to enhance patient care
Participation: Actively engage in care planning and interdisciplinary team meetings, addressing psychosocial aspects of care
Education:
Master’s degree in Social Work (MSW) from an accredited program, OR
Bachelor’s degree in Social Work with additional credentials that meet Medicare requirements
Experience:
Minimum of one (1) year of healthcare experience required
Hospice or home health experience preferred
Skills & Requirements:
Strong communication and organizational skills
Knowledge of hospice philosophy and end-of-life care
Current state social work licensure
Valid driver’s license with reliable, insured transportation
