Position Overview

Dependable Health Services is seeking a compassionate and dedicated Hospice Medical Social Worker to provide essential psychosocial support to hospice patients and their families. This role plays a vital part in helping individuals navigate end-of-life care with dignity, respect, and comfort.

Primary Responsibilities

  • Assessment: Evaluate the psychosocial needs of patients and families and communicate findings to the interdisciplinary care team

  • Counseling: Provide emotional support, crisis intervention, and coping strategies for patients and caregivers

  • Education: Assist with advance directives, explain patient rights, and connect families with community resources

  • Collaboration: Establish and maintain relationships with community agencies to enhance patient care

  • Participation: Actively engage in care planning and interdisciplinary team meetings, addressing psychosocial aspects of care

Qualifications

Education:

  • Master’s degree in Social Work (MSW) from an accredited program, OR

  • Bachelor’s degree in Social Work with additional credentials that meet Medicare requirements

Experience:

  • Minimum of one (1) year of healthcare experience required

  • Hospice or home health experience preferred

Skills & Requirements:

  • Strong communication and organizational skills

  • Knowledge of hospice philosophy and end-of-life care

  • Current state social work licensure

  • Valid driver’s license with reliable, insured transportation
