We are seeking a Medical Social Worker with a Master’s degree to join our dedicated hospice team in the beautiful Baker City. This is a meaningful opportunity to provide emotional support, guidance, and resource coordination for our patients in their most critical times.

Key Responsibilities:

• Provide comprehensive assessments of patients and families to identify psychosocial, financial, and emotional needs.
• Develop and implement individualized care plans based on patient needs and preferences.
• Offer counseling and support to patients and families dealing with end-of-life issues, grief, and loss.
• Coordinate with interdisciplinary teams to ensure holistic care and support.
• Assist families in navigating community resources and support systems.
• Advocate for patient and family rights and wishes within the care team.

Qualifications:

• Master’s Degree in Social Work from an accredited institution.
• Current licensure as a Social Worker in the state of Oregon.
• Experience in hospice or palliative care is highly preferred.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a collaborative team.
• Strong organizational and time management skills.
