Registered Nurse (RN) – Home Health

Location: Medford, OR and surrounding areas
Contract Type: 13-week travel assignment with possible extension
Shift: Day Shift | 5x8-hour (8:00 AM – 4:30 PM)
Start Date: November 17, 2025
Pay Rate: $2500 - $2800 gross weekly

Job Overview

Cambay Healthcare is seeking a skilled and compassionate Registered Nurse (Home Health) to join their dedicated care team. In this role, you will provide comprehensive nursing care to adult patients in their homes, focusing on assessment, education, and coordination of care to promote optimal health and recovery. Responsibilities & Duties

• Perform admission assessments , start of care , recertification , resumption of care , and routine visits .
• Deliver comprehensive skilled nursing services to adult patients in the home setting.
• Administer medications via intradermal, subcutaneous, intramuscular, and intravenous routes.
• Manage wound care, dressing changes, and infection control procedures.
• Provide patient and family education on disease management, medications, and treatment plans.
• Utilize EPIC and OASIS for charting and documentation.
• Conduct discharge planning and ensure smooth transitions of care.
• Collaborate with an interdisciplinary team to create and update individualized care plans.
• Maintain compliance with all agency, state, and federal guidelines.

Requirements

• Minimum Experience: 1 year of skilled Adult Home Health RN experience required.
• Certification: Current BLS (Basic Life Support) certification required.
• License: Active Oregon RN license (pending license accepted).
• Documentation: Proficiency with OASIS and EPIC charting required.
• Transportation: Must have reliable personal transportation for patient visits.
• Education: Graduate of an accredited nursing program (BSN preferred).
• Other: COVID vaccination series and one booster required by facility policy.

Schedule & Work Details

• Shift: 5x8-hour Day shifts (8:00 AM – 4:30 PM)
• Guaranteed Hours: Facility may call off up to one shift per pay period (every two weeks).
• Weekend Requirement: None
• Call Requirement: None
• Block Scheduling: Not available
• Patient Ratio: 1:1 (average 6 patients/day or 30 productivity points)
• Mileage Reimbursement: IRS standard rate
• Charting System: EPIC / OASIS
• Scrub Color: Any color scrubs allowed
• Parking: Free onsite

Preferred Skills

• Case Management experience
• Strong knowledge of infection control practices
• Experience with wound care and pain management
• Excellent communication and documentation skills
• Ability to work independently and manage time efficiently
