Job Description

Triboro Center is looking to hire a Wellness Coach to work Part-Time in Bronx, NY.
Key Responsibilities:
Develop and implement individualized wellness programs tailored to residents’ physical abilities, cognitive levels, and personal interests.
Conduct regular wellness assessments, monitoring progress, and adjusting programs as needed.
Lead group activities such as light exercise, mindfulness, stress-reduction, and wellness workshops.
Partner with rehabilitation, nursing, and dietary teams to promote healthy routines and continuity of care.
Provide motivational coaching and support to residents in areas such as mobility, nutrition awareness, social engagement, and emotional resilience.
Educate residents and families on wellness practices to support independence and overall health.
Collaborate with recreation staff to design engaging group activities that promote physical movement, social connection, and mental stimulation.
Maintain accurate records of resident participation, progress, and feedback.
Support a culture of holistic care in line with Triboro Center’s mission to enhance quality of life.
Qualifications:
Bachelor’s degree in Social Work, or related field (preferred CASAC).
Certification in substance abuse and mental health certification
Previous experience working with seniors, especially in long-term care and or drug rehabilitation.
Strong communication, motivational, and interpersonal skills.
Ability to adapt wellness programs to accommodate a wide range of physical and cognitive needs.
Compassionate, patient, and enthusiastic about improving residents’ lives.
